    Daniel Kahneman has introduced a new concept of pre mortem—to improve the quality of decisions. Great idea. Is akin to a Pre-delivery inspection carried out of a vehicle before it leaves the factory. Though all parts have been subjected to quality checks for compliance to specifications, the whole needs to be checked before it is shipped or handed to the customer.

    For the recommended premortem meeting, I suggest a preview be obtained from the participants to get a benchmark and anchor the meeting accordingly. The format needs to be simple. As Dr. Daniel already states, dissenters are shunned and perceived as disloyal, therefore the format needs to sent from a neutral party or on a hard copy where the traceability is minimized. This also gives time to the head/leader wanting to reflect and have a healthier discussion than the one which does again have an element of unstated dissent. The quality improvement effort can never, though, be complete.
